There are many moments in your Baldur’s Gate 3 experience where you might wonder if things can get any crazier. As you try to get rid of the tadpole that is giving you extraordinary powers, you will start doing things that you would have never thought possible. One of these things in Baldur’s Gate 3 is getting eye surgery from Volo, the world-renowned poet. Though most of us would never go through with this choice in real life, would it actually be worth it in BG3? In this guide, we will tell you what happens if you go through with the eye surgery that Volo wants to give you in Baldur’s Gate 3 and tell you what benefits you can get from it.

Effects of the Eye Surgery Volo Can Give You in BG3

After you rescue Volo from the Shattered Sanctum in the Goblin Camp, players will get to keep Volo in the camp for a little while, where he will offer to help the main character with their tadpole problem.

It is very clear that Volo has no idea what he is doing and, after a Long Rest, he will offer to extract the tadpole and do eye surgery on you. If you let him extract the tadpole, you will see that Volo is just gruesomely destroying your eye.

During this surgery, you can back out at any point. However, if you go through with the eye surgery Volo is giving you in BG3, you will get Volo’s Ersatz Eye, as a present, to put in your eye socket instead of your old, destroyed eye.

Though this might seem like a stupid choice, getting Volo’s Ersatz Eye will allow your main character to see Invisibility. This means that nothing will be able to hide from you ever again. The battle with Auntie Ethel is a lot simpler if you can see her from the start.

In regards to Auntie Ethel, you cannot get the eye surgery from Volo in BG3 if you already gave your eye to the hag. If you gave the eye with some other character, then you can get Volo’s one for your MC.

The only disadvantage you get from getting Volo’s Ersatz Eye is that Astarion, Karlach, and Lae’zel will disapprove of your choice.
